Description

The Eleventh Vermont Blueprint for Health Annual Conference explores how empowered patients, providers, and organizations are building energized, engaged, and activated communities of health. This year’s speakers include John Santa, MD, MPH, Medical Director for Consumer Reports, who oversees the Health Impact team of Consumer Health Choices and Kevin Grumbach, MD, Professor and Chair, UCSF Department of Family and Community Medicine and leading researcher in how primary care transformation is shaping and fitting into other national reforms including ACOs.

The Morning Sessions include:
